Tradewind Recruitment are currently recruiting for a Family Support Worker for our client in Merton.

The Family Wellbeing case practitioner's role is to be proactive and dynamic in building effective and meaningful relationships with children, families and other agencies, that facilitate high quality and comprehensive whole-family needs assessment and to lead, develop and implement a tailored support plan together with the family to meet the identified needs.

Main duites:

  • Lead on carrying out whole family assessments and undertake the development of action plans with a high level of competency
  • Take a lead in working with families, whose needs are of a high complexity that include a wide range of factors that require a specific and individualised response through delivery of group based or 1 to 1 interventions/programmes
  • Be the lead contact with other identified agencies, such as Education, Children's Centres, Community and Voluntary sector, children and adults Social Care, Housing, Health and Criminal Justice agencies. Coordinating the gathering and sharing of information and chairing multi agency meetings
  • Developing excellent working relationships leading to improved intervention and long term outcomes for families and their children/young people
  • Lead and deliver evidenced based interventions through a case work model in the family home, co-ordinating and managing planned targeted interventions in a structured environment, offering both support and challenge to change family behaviours, underpinned by Merton's Early Help practice model and motivational interviewing techniques

Skills and experiences:

  • A minimum of a level 3 qualification in a relative field eg early years, health, social care, youth work etc
  • Local Authority expereince of working as a Family Support Worker
  • Experience of working with children, young people and families with a range of multiple and complex needs and risk factors, with the ability to engage and empower individuals to bring about change and achieve positive outcomes.
  • Knowledge of policies, statutory regulations and guidance relating to child development, safeguarding and child protection, SEND, youth crime and antisocial behaviour and how they are applied in practice with an ability to work appropriately within these
  • An understanding of Child and Adolescent Safeguarding and child protection policy, and an ability to gather and analyse information to make assessments on vulnerable children, young people and their families, including those with diverse and complex needs and to formulate and implement subsequent family plans based upon SMART objectives which inform ongoing interventions
