Tradewind Social Care are looking to recruit a Qualified Social Worker to join a busy Hospital Discharge Team in the London Borough of Lambeth.

The main purpose of the role is deliver a high quality initial assessment and support service for service users in hospital, which will include initial safeguarding enquires and the aim to work towards the discharge of the patient.

Main Duties:

To undertake assessments of need and construct care plans and packages to meet the needs of service users and carers

To participate in team arrangements to provide an urgent or emergency response, during working hours as required

To complete reports as required by departmental procedures

To work in partnership with service users and carers using a person-centred approach

To attend and participate in meetings regarding service users, e.g. case conferences, network meetings and reviews

To act as a representative at departmental working groups and panels as appropriate

To be fully aware of the principles of safeguarding as they apply to vulnerable adults in relation to your work role

To act as the Investigating Officer in accordance with the role and tasks specified in Liverpool's Safeguarding Vulnerable Adults Policy and Procedures

Essential skills & experience needed:

Experience of risk assessment, safeguarding investigations and understand the need to keep vulnerable adults safe
Good communication skills and the ability to relate to adults, carers and families
Ability to write appropriate reports and case records

Ability to build effective professional relationships with service users

Job Requirements:
An enhanced DBS that is of satisfactory level is essential
Must have a minimum of 2 years experience in a similar role

Must be Social Work qualified with a valid HCPC registration

Clear UK Driving Licence
