There is great opportunity for a Qualified Social Worker to work within a Hospital setting in the London Borough of Merton


The main purpose of the role is to provide Social Work to Hounslow residents and carers and undertake Social Care Assessments and to identify their needs and desired outcomes which places the resident at the centre of all interventions and giving them choice and control over the delivery of services. To protect the rights and promote the interests of Hounslow residents and their carers to ensure they are kept safe, in line with the Council's and Pan London Safeguarding Policy including investigations, contributing to strategy meetings and actions.

Main Duties:

To prepare, attend and present reports for Case Conferences, Panels, Tribunals, review meetings and Court

Carry out assessments, casework, support plans and discharge planning as directed

Use a range of therapeutic approaches in your work to enhance the coping skills, independence, choice and control of the vulnerable resident, their family and carers when working with vulnerable residents their families and carers

Include cost/best value considerations and commission creatively for outcomes in order to deliver individualised, person centred support

Adhere to the Mental Capacity Act, undertaking mental capacity assessments where appropriate, referring to an IMCA as per the Act or undertaking Best Interests under the Deprivation of Liberty safeguards

Monitor and review support plans, involving all stakeholders and ensuring the vulnerable adult is at the centre of all interventions and process ensuring that plans meet assessed eligible needs and service standards and are undertaken within statutory timescales

Mobilise and coordinate available resources within the department and those provided by other services, departments, agencies and wider community

Liaise effectively with hospital, community groups and statutory organisations i.e. finance, placements, legal, NHS professionals, voluntary and provider organisations, housing in terms of assessment, casework and support planning

Form robust and effective working relationships with colleagues across the Council and in partner organisations

Essential Skills & experience needed:

Ability to work with communities, partner organisations and voluntary sector to maximise social capital for vulnerable residents and ensure support is culturally appropriate

Knowledge and understanding of relevant social services legislation, particularly 1990 NHS and Community Care Act and how this relates to the statutory role of social care

Job Requirements:
An enhanced DBS that is of satisfactory level is essential
Must have a minimum of 2 years' experience

Must be Social Work qualified with a valid HCPC registration
