Tradewind are currently recruiting for a Senior Support Worker needed to provide outreach support work for clients in Hammersmith and Fulham at risk of rough sleeping.

Clients live in shared accommodation managed by Hestia, with staff support on site 24/7. The support provided is short term (6 months -2years) with staff supporting clients to achieve their goals with the aim being to promote independence. The Service is designed to support individuals to help them overcome their challenges and live as independently as possible. We have 12 clients at any one time. The Service User is an individual so the support available will be customised to their particular situation and will last for as long as it is needed.

Main duties:

  • Empower a caseload of clients with a range of complex health and support needs and their carers to manage their health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations, working in partnership with our subcontractors as required in order to achieve this, under the direction of the Team Manager
  • Manage health and safety, repairs and maintenance of the premises
  • To collect rents, service charges and manage arrears proactively by following policy, protocol and procedure
  • To manage voids across the service and ensure referrals, assessments and service reviews are effective and timely
  • To clearly articulate the aims and objectives, pathway and service model to people using the service so they can fully participate in and co-produce their support
  • Empower service users to engage effectively in their liaisons with professionals within the community such as social work teams, housing providers, health professionals, JCP etc
  • Work flexibly as part of a team to ensure service coverage and that contractual obligations are met
  • Develop co-produced group activities, peer support networks and other community-based approaches to empowering service users to achieve their goals and aspirations and to encourage service users to access these opportunities
  • Actively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are followed up promptly and in line with procedures
  • Participate in the core work of the service, providing support to colleagues, carrying out tasks allocated by the team manager and using initiative to identify and deal with additional tasks as they arise

Skills and experiences:

  • Ability to work with people in a person-centred way
  • An understanding of support needs in mental health
  • Knowledge of the behaviour and symptoms associated with mental health and psychosis
  • A detailed understanding of key-working, personalised support, and coproduction
  • An awareness of safeguarding procedures and risk assessment principles
  • Commitment to working within an Equality & Diversity Framework
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately
