There is currently a fabulous opportunity for an experienced Social Worker to join the Transition Team based in the London Borough of Southwark Council.

The post holder will play a key role in improving transition for children and young people aged between 14-25 with a learning disability, physical disability or sensory impairment and their families and carers. The role will include casework with young adults, person centred planning as well as responsibilities for developmental work including the active take up and promotion of individualised budgets.

Main Duties:

To carry a caseload of complex cases involving young people going through transition from Children to Adults' Services

To undertake statutory duties including casework responsibilities in accordance with legislative requirements for the delivery of children's services and community care

To plan the transition for young people between children's and adults' services in order to ensure consistency and to minimise the anxiety for the young person and their carer(s) using relevant tools including PCP plans, linking with Children's Services, Schools, Connexions, LSC and SEN and all other relevant agencies

To contribute to the development of interdepartmental policy about the transition of young people

To manage the hand over of case work responsibility to the Learning Disabilities Social Work Team, or the Physical and Sensory Impairment Team, according to agreed timescales at and as directed by your line manager

To work in partnership disabled young people, parents/those with parental responsibility and carers

To prepare young disabled people for transition to adulthood. To implement care plans and help young people who will cease to be looked after prepare for independent living

To be responsible for individual client case planning in line with the professional standards of the department.

To provide a preventative and rehabilitative service within the criteria of the Children Act, facilitating appropriate resources where necessary to enable children to remain with their own families.

To Monitor and supervise the well being of disabled children and young people in need, disabled children looked after and disabled children whose names have been placed on the child protection register according to statutory and local guidelines and procedures

To assess situations where children are referred as being in need of accommodation or care provision, and take appropriate action based on evidence

To aim that child should remain with their own families wherever possible. Where it is legally upheld that natural family circumstances have irretrievably broken down, to plan a secure and permanent home for children, ensuring that they have clear information about their own background and contact with natural relatives wherever possible.

Essential skills & experience needed:

Proven ability to undertake assessments, write complex care plans and reports

A working knowledge of Children's Legislation that relates to or has an impact on the Transitions Service

Job requirements:
An enhanced DBS that is of satisfactory level is essential
All applicants need to be social work qualified with current HCPC registration

Must have a minimum of 2 years experience working within Children's services
