We have a great new opportunity for an experienced Support Planner to join their Adult Social Care Service within East London.

You will work together with Housing with Care staff to review and develop person centred support plans and associated risk assessments in line with best practice and regulatory requirements. They will work within Housing with Care settings, visiting service users, and drawing up plans in conjunction with the service users, their friends/relatives and care support staff.

The responsibilities of an Support Planner includes:

  • To actively listen to service users, relatives/friends, through verbal and non-verbal communication, recording people's preferences and responding to their wishes as appropriate
  • Using person centred approaches, develop outcome focused support plans to meet identified need using a strengths based approach
  • Support individuals and staff to identify potential risks and find solutions through carrying out a range of risk assessments, dependant on individual need
  • Assist people to find creative solutions to meet their identified need and ensure that this is recorded in a manner that meets accessible information standards
  • To work with all relevant people and organisations in support of a person to ensure needs are met effectively, meeting identified timescales
  • To ensure a high level of data quality for all information recorded, ensuring that it is completed in a manner that is thorough, accurate and timely

Requirements of an Support Planner:

  • 2 years recent experience of working in a CQC regulated care setting with people who have complex needs including older people, people with dementia, people with physical disabilities, people with learning disabilities
  • Ability to assess service users' needs, through a person-centred approach, empowering service users to express their views and achieve their potential to ensure identified outcomes are met
  • Ability to write clear and factual care notes/reports and follow written care and service plans. Able to produce written reports which have structure, style and content relevant to their purpose
  • An enhanced DBS that is of satisfactory level is essential

Salary: Up to £22.03 p/h, based on experience
